Our Bill Payment Patient Portal

Office Policies

We have several policies and procedures that we have established to ensure the highest quality of service and care during your visit. If you have any questions or concerns about these policies, we ask that you contact our office prior to your visit.

What to Bring

We require that you bring the following items to your visit:

  • Insurance card
  • Identification card
  • Co-pay for insurance patients, full payment for self-pay patients
  • Credit card for patients with high-deductible plans
  • Referral or Insurance Authorization if required by insurance company.
  • A guardian if you are under 18 years old

If you do not bring the above items with you, we reserve the right to reschedule or cancel your appointment.

Patient Forms

Insurance Cards

All patients who will be utilizing their insurance coverage are required to bring their current insurance identification card to each office visit. If the patient does not have the card then the patient will have to cancel or be seen as a self-pay patient. If we have to cancel your appointment for this reason, we reserve the right to assess a cancellation fee.

Co-pays

Patients who have co-pays as part of an agreement with their insurance carrier will be required to pay on the day of service. It is the responsibility of the patient to bring appropriate means of payment: cash, check or credit card. We accept American Express, Mastercard, and Visa. A service fee will be charged to any patient who does not remit this payment on the day of service.

Referrals and Authorizations

Referrals are a contractual agreement between patients and their insurance carrier. If a referral is required before a visit, it is the patient’s responsibility to obtain one from their primary care doctor and to bring a copy to the specialist. If the patient does not obtain the referral and bring a copy of the referral with him, the appointment may be canceled and the patient will have to reschedule. If you would like to be seen without a referral, you will need to sign a release in which you agree to pay for the visit in full if the referral is not produced. 

Legal Guardians

All minors are required to have a parent or legal guardian present with them for their initial visit. Their presence is also required for any surgical procedures or new courses of treatment.

Cancellations and No Shows

Prior to an appointment our office will attempt to contact the patient by phone as a reminder of the appointment. If the patient is unable to keep their appointment they are required to give at least 24 hours notice. If notice is not given on time, the patient may be charged $25.00. Anything less than 24 hours does not allow us sufficient time to release that appointment to another patient.

We accept all the major insurance plans. This is a partial list of the insurances we accept. The list my change, please confirm insurance information with office when you make your appointment.
